Fundamentals in Incident Investigations
Fundamentals in Incident Investigations provides investigators with the knowledge and skills to: Conduct a thorough and effective incident investigation; identify, collect and catalog relevant evidence to support analysis; analyze collected data to determine causes of the incident; and effectively report findings
Human Factors Analysis in Incident Investigations
This course introduces the fundamentals of analyzing human error in problem solving and incident investigations. It explores the theory behind human error and teaches investigators not only how to identify human error, but more importantly, to determine why the error was made. In this course, students will learn how to gather and analyze evidence specific to the factors related to human error. They will be introduced to tools and methods that will enable them to consistently identify, define, and classify the preconditions that commonly exist at the supervisory and organizational level that often result in poor decision making by employees. The training will be reinforced with hands-on exercises to emphasize course objectives and allow the student to become comfortable with using the tools and methods provided. Upon completing this training, students will have the knowledge to consistently identify the latent conditions that influence decisions which lead to human failure.
Facilitated Investigation Workshop
The Facilitated Investigation Workshop is a 1-day practical exercise in the form of an incident investigation where the instructor serves as a member of the investigation team providing guidance where needed. Students are given the “what, when and where” of a realistic incident to investigate and work as a team applying practical skills to: Identify and collect evidence; conduct analysis; determine causal factors; and identify potential solutions. This facilitated workshop provides investigators with an opportunity to apply incident investigation and Human Factors Analysis methods, techniques and skills acquired during previous training.
